26-27 Staff BTS Grant Reimbursement Request

Instructions:

Please fill out completely with description of reimbursement and breakdown of expenses.
Attach a file/picture/scan of each receipt or supporting documentation.
- Please do NOT use "live" iphone photos as attachments, as still/normal photos work better.
- Consider using Word or PDF to lump multiple receipts for the same vendor/expense category into one attachment.
- If you have more than 5 vendor/expense categories please submit a second reimbursement request.

As a sales tax exempt organization, we cannot reimburse sales tax. Please give all vendors a copy of our sales tax exemption form to avoid being charged sales tax when you make your purchase.
